A 28-year-old patient consulted for this pigmented lesion on his right forearm.

Dermoscopy revealed:

• multiple patches of pigmentation
• without background pigmentation

Clinical and dermoscopic examination were in favor of an agminated nevus.

Differential diagnosis is nevus spilus: this one includes a tan background patch which is not present in agminated nevus.

Compound melanocytic nevus

Dermoscopy revealed a central part with exophytic papillary structures and a peripheral typical reticular pattern in favor of a benign compound melanocytic nevus.

Hypopigmentation

Hypopigmented areas are frequently observed in melanocytic nevi.
In this case, hypopigmentation was associated with a typical reticular pattern.
In melanocytic nevi, hypopigmentation is often patchy, multifocal and perifollicular.
